Psychological Evaluations

What is a psychological evaluation?
A psychological evaluation is a comprehensive evaluation of a person’s emotional, behavioral, cognitive, and social complaints. The evaluation involves the utilization of tools and techniques to gather information that can help diagnose mental health conditions, understand your strengths and weaknesses, and develop a treatment plan to ultimately improve your overall functioning. The evaluation is completed by a licensed psychologist.
The purpose of a psychological evaluation or psychological assessment:
Integrative Health offers in person psychological evaluations across the age spectrum to aid in differential diagnosis. We provide psychological assessment services to patients with concerns related to Autism Spectrum Disorder (ASD), Learning Disorder, Attention-Deficit/Hyperactivity Disorder (ADHD), personality disorders, and mood disorder (depression, bipolar, etc.)
Our evaluation process:
- Meet with the provider to review intake form and documentation (e.g., school records, medical records, psychiatric records, etc.).
- Complete 1-3 hours of standardizes tests (same day as the intake)
- With the patient’s consent, collateral information is obtained either during or after the intake from sources such as teachers, physicians, psychotherapists, caregivers, etc.
- During the initial appointment, your provider will schedule a follow up/feedback/results appointment. This can be done via telehealth or in person. The provider will communicate findings, diagnosis, prognosis, and treatment recommendations. At this time, a written report will be provided to you and can be shared with your treating provider(s). If requested, the provider will send you and your care team a copy of the report
Preparing for your psychological evaluation appointment:
- Try and obtain a good night’s sleep
- Eat a hearty breakfast
- Take all of your medications as usual unless you are directly told otherwise by your doctor. If taking a stimulant medication, ask your doctor if it is ok you do not take the medication during the first appointment as this may impact your test results
- If you wear glasses, contact lenses, or hearing aid, please bring them with you
- Please upload the following records into the Client Portal if possible: academic records (e.g., IEP), previous psychological or neuropsychological evaluations, MRI, CT, recent lab work
- You will need to sign a release form given the doctor permission to share your results with another healthcare provider outside of Integrative Health, so please have their contact information available
- If a family member attends the appointment with you, the doctor will ask for your permission to interview them. They may be asked to complete forms as well.
- If you are not feeling well, please reschedule the appointment
- Please bring snacks and dress comfortably
